Record the organization, business unit, facility, executive sponsor, process owner, and submission date. Define the trigger, endpoint, product or service family, and core function.

02 / CAPTURE THE BASELINE
Measure before prioritizing.
Collect lead time, cycle time, work-in-progress, first-pass yield, monthly scrap or rework cost, on-time delivery, and operating headcount. Record the target and data-source confidence alongside each estimate.
Rate the seven Lean wastes on a 1–5 severity scale: overproduction, waiting, transport, over-processing, inventory, motion, and defects. Add notes to explain the operational impact.
03 / ALIGN & SCORE
Connect improvement to business priorities.
Document customer impact, financial and strategic relevance, team readiness, and safety or compliance risks.
The consultant-completed scorecard weights customer value and lead-time impact at 30%, financial and waste-reduction potential at 25%, strategic alignment at 25%, and implementation feasibility and readiness at 20%.
Recommendations range from immediate Kaizen work to a detailed VSM diagnostic, local standardized work or 5S, or deferral.
